Insurance
I am an out-of-network provider. I do not belong to any insurance plans, preferred provider organizations (PPOs), health maintenance organizations (HMOs), Medicare or Medicaid.. I do not participate in Medicare or any other government health insurance programs.
Depending on your current health insurance provider or employee benefit plan, my services may be covered in part. Please contact your provider to verify how your plan compensates you for out-of-network psychotherapy services.
I recommend asking these questions to your insurance provider to help determine your benefits:
- Does my health insurance plan include behavioral health benefits?
- Does my plan have an out-of-network benefit?
- Do I need a referral from my primary care physician for services to be covered?
- Do I have a deductible? If so, what is it and have I met it yet?
- Presuming I have an ICD-10 diagnosis, what % of CPT 90804 (50 minute sessions) is covered?
- Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
Payment
I accept cash, checks, and all major credit cards as payment.
Cancellation Policy
If you are unable to attend a session, please cancel at least 24 hours beforehand.
